This registration session will allow you to complete contact information, sign up for participation, and submit payment.

- AYF Medical Clearance form OR Physical dated this 2023 year
- End of year report card showing all 4 terms
- Birth Certificate (New Players and Cheerleaders only)

Custom Football Uniforms
Custom Football Uniforms are included with each registration. Orders must be in and paid by June 23, 2023. Once you have paid in full, please email Amanda, the jersey number you would like to reserve with your child's information.
